Covid-19: Stick to paracetemol, coronavirus patients recovering at home told

If you're looking for relief from coronavirus symptoms, take paracetamol rather than ibuprofen, and avoid non-evidence-based treatments being promoted on social media, health experts recommend.

Australian doctors typically treat patients with a fever – one of the key symptoms of COVID-19 – with the painkiller paracetamol, often sold as Panadol.

Paracetamol provides quick relief from symptoms and is relatively safe.

Sick? Stick to panadol, experts advise.

In other countries, doctors regularly use the anti-inflammatory ibuprofen, often sold here as Nurofen, instead.

There is some concern among scientists that ibuprofen may worsen the effect of the virus by suppressing the immune system.

There is no strong evidence of this. But out of an abundance of caution the World Health Organisation is now encouraging people to take paracetamol rather than ibuprofen if they have COVID-19.

"We recommend using rather paracetamol, and do not use ibuprofen as a self-medication. That's important," WHO spokesman Christian Lindmeier said this week.

The advice follows an onslaught of misinformation about coronavirus treatments on social media.

Australians with coronavirus recovering at home are being urged by infectious disease doctors to use scientifically proven medication, like paracetamol, to ease symptoms including elevated temperatures.

The WHO's announcement came after Olivier Veran, a qualified doctor and France's Health Minister, tweeted on Saturday: “The taking of anti-inflammatories [ibuprofen, cortisone … ] could be a factor in aggravating the infection.

"In case of fever, take paracetamol. If you are already taking anti-inflammatory drugs, ask your doctor’s advice.”

Inflammation is a sign of the body's immune system fighting back against a virus. As immune cells rush to the site of the infection, the infected tissues heat up and swell.

Dampening that process means you cut the symptoms of an infection, but you also potentially slow down your body's attempts to kill the virus.

"Instead of putting out the fire quickly by letting the immune system go full-bore, you’re putting a handbrake on it," says Professor Stuart Tangye, head of immunity and inflammation research at the Garvan Institute in Sydney.

“Potentially, it will take longer to clear the virus – and potentially you’re infectious for a longer period of time.”

Associate Professor Steven Tong, an infectious-disease physician at the Peter Doherty Institute for Infection and Immunity, said that because coronavirus was such a new disease, there was not enough evidence to prove taking ibuprofen was harmful.

"I think where he [Mr Veran] has come from was that if you suppress the immune system that might be a bad thing, so people who are on steroids and things like that. I suspect he's kind conflated that with non-steroidal anti-inflammatories as well."

Professor Tong's advice for patients is to take paracetamol because it is highly effective at lowering elevated temperatures.

"It really provides some symptom relief. Keep up the fluids, take paracetamol and get some rest, that is what I suggest."

Professor Bill Rawlinson, senior medical virologist at NSW Health Pathology, said that when patients were diagnosed with influenza they were urged not to take non-steroidal anti-inflammatories, like aspirin, due to the risk of Reye's syndrome, a rare disorder that causes brain and liver damage.

Concerning coronavirus, Professor Rawlinson stressed there was not enough data to prove ibuprofen was dangerous. His advice, however, was to take paracetamol.

"It [paracetamol] is a good one for controlling fever so that's sensible," he said.

Those recovering from coronavirus at home are urged to self-isolate, refrain from vigorous exercise, monitor symptoms and avoid unproven remedies such as vitamin or herbal boosters claiming to prevent or cure coronavirus.

Unlike influenza, where patients could take Tamiflu to halt the disease, there is no effective treatment for COVID-19 yet. It is up to the infected person's immune system to fight the virus.

"If you took paracetamol, or even if you took ibuprofen, it is unlikely to make a difference as to how long you're sick for or how sick you are going to get," Professor Tong said.

He is working alongside Brisbane scientists on a series of revolutionary clinical research trials examining the effectiveness of anti-HIV medicine Kaletra and another drug called hydroxychloroquine, commonly used to treat malaria.

Both medications have killed the virus in test tube studies, with the next stage set to be a clinical trial on humans once funding can be secured.

Meanwhile, the Royal Australian College of General Practitioners has warned Australians to be mindful of false or misleading medical advice on social media.

Covid-19 FAQ

Coronavirus: No-Panic Helpguide
By 1mg

Here are the answers to the most common queries about the Novel Coronavirus based on our discussion with the various experts from the reputed institutes and analysis of the CDC, WHO, and MoHFW guidelines.

Prepare carefully, but don’t panic

A) Basics first
COVID-19 is an infectious condition, which means it can be spread, directly or indirectly, from one person to another
- Involves your upper respiratory tract (nose, throat, airways, lungs).
- Caused by the newly discovered coronavirus, called as novel coronavirus, first identified in Wuhan, China, in December 2019.

B) How deadly is Coronavirus?
Coronavirus has “high infectivity but low mortality”
- Mortality rate ranges between 2-3%. It is significantly less severe than 2003 SARS (MR: 10%) or 2012 MERS (MR: 35%) outbreaks.
- Risk of death is only higher in older people (above an age of ~60 years) and people with pre-existing health conditions.

C) Why is there so much panic?
The lack of verified facts and floating rumours are to be blamed for this panic. The most important thing is when a virus is new, we don’t know how it may affect people.

D) Will I die if I catch the disease?
- Almost 80% of people have mild symptoms, and recover from the disease in 2 weeks.
- Most of the symptoms can be treated with timely medical care.

Total no. of confirmed cases 121,061
No. of deaths 4,368
No. of recovered cases 66,216
No. of active cases, receiving medical care 57,463

Source: John Hopkins Center for System Science and Engineering (Live dashboard), as reported on March 11, 2020.

E) Who is getting sick and how sick?
- Older people are twice as likely to have serious COVID-19 illness.
- Most coronavirus led illnesses are generally mild, especially for children and young adults.

F) Are children also at risk of coronavirus infection?
COVID-19 appears to be relatively rare and mild in children.

- Just over 2% of cases were under 18 years of age.
- Of these, fewer than 3% developed severe or critical disease.

G) So, how exactly does the Coronavirus spread?
An infected person can spread the infection to a healthy person
- through eye, nose, and mouth, via droplets produced on coughing or sneezing.
- close contact with infected person.
- contact with contaminated surfaces, objects, or items of personal use.

H) Can the novel coronavirus be passed on through food?
No
There is no such evidence as yet.
- experience with other coronaviruses like SARS and MERS suggest that people do not get infected through food.

I) Can eating chicken or eggs cause Coronavirus?
No
There is no such evidence as yet.
- The novel coronavirus is not known to spread directly through poultry products. But, experts say it can be a good option to have only properly cooked meat.

J) Do I have coronavirus if I am coughing or sneezing?
You can suspect to have coronavirus, only if You have symptoms (fever, cough, shortness of breath) + any out of these:
- Travel history to a coronavirus-affected area (like China, Iran, Italy, Republic of Korea, etc.).
- Close contact with such a person.
- Visiting a healthcare facility/ labs where Coronavirus patients are being taken care of.

K) Track what your symptoms are
Coronavirus can get you symptoms like fever, cough, shortness of breath. They can be almost similar to common cold or flu and may appear 2-14 days of exposure. Severity of illness can vary in different people from mild to severe symptoms.
If you have a fever, cold, or sore throat, don’t panic. Take good care of yourself:
- Inhale steam 2-3 times a day to clear off congestion
- Stay hydrated and get adequate rest*
- Washing your hands frequently can reduce the spread of the virus
- Take medicines as and when advised by the doctor
- Visit a doctor if condition worsens or does not get better with time

L) Cold Vs. Flu Vs. Coronavirus
If you have a sore throat, it’s more likely a cold than flu or coronavirus, in general.

Cold Flu Coronavirus Time between catching the virus and beginning to show symptoms 1-3 days 1-4 days 2-14 days Symptom onset Gradual Abrupt Gradual How long do symptoms last 7-12 days 3-7 days Mild cases:~2 weeks. Severe or critical disease: 3-6 weeks Major symptoms Fever Sometimes Common Common Runny nose Common Sometimes Less Common Sore throat Common Sometimes Less Common Cough Common Sometimes Common Rare; if occurs, mild Body Ache Common Difficulty Breathing Rare Rare Common Less Common 14

M) Should I be tested for coronavirus if I have high grade fever, cough and difficulty in breathing? Or can I get a coronavirus test done just for the sake of satisfaction?
No.
- Testing for coronavirus will only be done as recommended by the doctors only if someone falls under the definition of "suspected case".

N) Why can only designated labs test coronavirus? Why can't I get a home sample collection done for coronavirus testing?
- It is because the virus is highly contagious and can only be tested by special labs equipped to handle such contagious pathogens. As of now, only labs with biosafety level 4 (BSL4) can handle the COVID-19 samples.

O) Can a person infected with coronavirus recover completely and be no more infectious?
Yes!
- In fact, about 80% of people have recovered from the disease without needing special treatment.

P) Can coronavirus be treated?
Yes!
- Symptom-based medical care is given and has proven really effective.
- There is no specific treatment or vaccine available for novel coronavirus as yet.

Q) Can eating garlic prevent infection with the coronavirus?
- There is no science-based evidence that proves its ability to protect against the coronavirus.

R) Will warm weather stop the outbreak of COVID-19?
- As per the experts in the field, heat and warm weather is likely to decrease the spread of COVID-19.
S) How long does the coronavirus survive over the surfaces or objects?
- Survives up to 8-10 hours over porous surfaces (like paper, untreated wood, cardboard, sponge and fabric) and a little more than this over nonporous surfaces (like glass, plastics, metals, varnished wood).

T) What type of mask should one wear to protect against the coronavirus?
- The 3-layer disposable surgical masks are good enough to contain the virus.
- N-95 or N-99 masks are not mandated.

U) Should the coronavirus outbreak concern you about your pets or other animals?
To date, no case has been reported of pets or other animals becoming sick with COVID-19.
- We still recommend washing your hands before and after you interact with your pets as you may not know if some infected droplets get on your pet's fur.

V) Major concern: How easily does it spread?
- The virus that causes COVID-19 seems to be spreading easily and continually.
- Community spread is seen only in some affected geographic areas like China, Republic of Korea, Iran, Italy, Hong Kong, etc.

W) 3Ps: All it takes to defeat Coronavirus!
Channel your worries into these 3 actionable goals:
1. Protect yourselves.
2. Protect your loved ones.
3. Protect your community.

Protect yourselves:
- Wash your hand regularly with plenty of soap and water.
Keep an alcohol-based sanitizer ready for times when soap and water are not available.
Don’t touch your eyes, mouth or nose with unclean hands.
Keep your distance of at least 1 meter from anyone coughing or sneezing.
Follow no-touch greeting for no germs contracted. Prefer “namaste” or hand waving over handshake.

Protect your loved ones:
Don’t sneeze or cough into your hands. Use a tissue and throw it away immediately, or sneeze in the inner side of your elbow.
Don’t travel or visit crowded places if you’re sick.
Do wear a mask if you’re sick and also if you are taking care of someone with the symptoms.

Protect your community:
- If you feel unwell, seek medical attention.
- If you have a fever, cough, or difficulty breathing, stay indoors. Call healthcare professionals and follow their advice.
- Do not share “just any forwarded message”. Only share authentic information coming through medical experts.

Covid-19 Lockdown - Msia as in China

Finally. Lockdown in Malaysia. Anyone need any advice on how to survive a lockdown just let me know. I have been in quarantine and lockdown for over 4 weeks now in Beijing. :)

Important things you need to get.

1. Stock up panadol and medicine. It is a bad time to go to the hospital or clinic.

2. Get a thermometer to measure your temperature twice daily, in the morning and in the evening. Record it down.

Just to prepare some of you in Malaysia what's gonna happen.

1. All your favourite restaurants and mamaks will be closed.

2. Supermarkets will still be open for food and toilet paper. But I would suggest that if possible only go out once or twice a week just to get food. Go during off peak time so you get less crowds.

3. Most malls will be closed too so no point going to malls.

4. Avoid going to parks for jogging, a lot people running there and sweating without masks.

5. Avoid taking public transportation like buses and trains. The virus spreads easier in confined spaces especially with air condition.

6. Avoid ALL possible crowded areas if possible.

7. Always if possible wear a mask. People in the western countries tell you to only wear a mask if you are sick it is because they do not have enough mask in their country. Wear it for yourself and others, you got nothing to lose. When you arrive home, remove your mask without touching your face and throw it away into a plastic bag and dispose of it. And wash your hands with soap.

8. Disposable latex gloves are cheap in Malaysia so wear them when you go to the supermarket and banks to handle bank notes to other people. Also when you carry your groceries and food items. Dispose them the same way as your mask, then wash your hands with soap.

9. Disinfect your clothes after you come back from outside. Wash your hands first, then if possible also wipe the inside of your car steering wheel and door handles. Anything that you have touched need to be cleaned.

10. Don't forget to disinfect your mobile phone too especially the screen and covers. And for the ladies your handbags.

11. Try to do food order delivery as most of the restaurants are close, some might still open for deliveries.

12. If you wear any shoes, sandals or slippers to go outside please remember to disinfect them too and wash your feet with soap.

13. In fact, I take a long shower after I come back home everytime just to be safe. The clothes immediately goes into the washing machine to wash.

That's all for now, have a happy lockdown Malaysia.

Please share this post to anyone who need to know. God bless and stay safe everyone.

Recite Psalm 91 with all your might and spiritual strength

Psalm 91
1 He who dwells in the shelter of the Most High will rest in the shadow of the Almighty.
2 I will say of the LORD, "He is my refuge and my fortress, my God, in whom I trust."
3 Surely he will save you from the fowler's snare and from the deadly pestilence.
4 He will cover you with his feathers, and under his wings you will find refuge; his faithfulness will be your shield and rampart.
5 You will not fear the terror of night, nor the arrow that flies by day,
6 nor the pestilence that stalks in the darkness, nor the plague that destroys at midday.
7 A thousand may fall at your side, ten thousand at your right hand, but it will not come near you.
8 You will only observe with your eyes and see the punishment of the wicked.
9 If you make the Most High your dwelling-- even the LORD, who is my refuge--
10 then no harm will befall you, no disaster will come near your tent.
11 For he will command his angels concerning you to guard you in all your ways;
12 they will lift you up in their hands, so that you will not strike your foot against a stone.
13 You will tread upon the lion and the cobra; you will trample the great lion and the serpent.
14 "Because he loves me," says the LORD, "I will rescue him; I will protect him, for he acknowledges my name.
15 He will call upon me, and I will answer him; I will be with him in trouble, I will deliver him and honor him.
16 With long life will I satisfy him and show him my salvation."

Covid-19 Chinese Msian & Italian Experience

I got this from a friend which I believe is a practical advice to share on adopting new daily habits to prevent COVID-19 it it gets worst.

From a Malaysian stuck in China

Covid-19 Pandemic

For those who didn't know, I've been in China Chongqing all these while when this circus started in end Jan. The whole world was watching China then, and now that China have it under control, we are watching this whole panic thing happening around the world. Maybe I can share my experience how I kept myself safe.

Well first of all, it was not just Hubei and Wuhan that was in lockdown. Basically the whole of China was in lockdown. So the people of Hubei and Wuhan cannot leave their city. While in CQ we can't leave our residential compound. Yup, you read me right. We are given a permit to allow 1 pax from 1 household to exit and enter the residential compound every 2 or 3 days to get essential stuff, depending on how serious the situation is in your area. Everyone needs to get their body temperature checked upon entering any building at any time. And when I say essential stuff it really is essential stuff, back then during the curfew only markets, supermarkets, pharmacies, designated clinics and such are allowed to operate.  So no hanging around,  wandering around the streets aimlessly coz nothing else is open. And if you are wandering around, you'll be strongly advised by the patrolling police to get back home.

This curfew has been for 40days. So yes, we (basically the whole of China) have all been in a curfew/lockdown for that long. And that's billions of people we are talking about. It may be enforced by law, but everyone is willingly and voluntarily isolating themselves. No school, no work! This really is THE WAY TO STOP THE SPREADING!

Isolation and quarantined is violating human rights? Please, what's human right if you end up 6ft under.

And so, if I needed to go out to get essentials this is what I did, and still am doing. I usually just get my groceries that can last 1-2weeks, just so I don't need to go out too often and lower my risk of contraction.  I don't stock up on anything, esp toilet paper ahem...

1. Face mask on (this is strongly enforced here, one can get arrested if they refused to)
2. Have my hand sanitizer and a small bottle of 75% alcohol spray with me.
3. Once I leave my house, If I touch the lift buttons or anything else, I'll make sure to sanitize my hands before I get in my car and start touching everything else. (I refuse to use a tissue or glove cause I don't want to create unnecessary rubbish, I can live with dry hand skin)
* So if you need to take the public transport and need to hold the handles/poles, just make sure you sanitise your hands as soon as you leave the bus/ train.
4. Go about getting my stuff. I don't touch my face especially my eyes nose and mouth when I'm out. If I really need to rub my eyes I will sanitise my hands [see the pattern here?] and use the inner part of my shirt to rub my eyes.
5. Done with shopping. Place stuff in car and sanitise hands.
6. Get home. Wash hands with soap and water, take off mask, bin it or hang it somewhere to air for reuse. Wash hands again!
7. Change my clothes and toss it directly into the washing machine, wash with 60℃ water. Hang my winter jackets in the balcony to air. Shower immediately.
8. Disinfect my groceries, those with packaging and my reusable shopping bags.
9. Disinfect car keys, door handle & lock and my handphones! HANDPHONES PEOPLE!

So, this is my SOP. It really is fairly simple.

1. MASK MASK MASK!!! Seriously! You are protecting yourself and everyone around you. It is contagious even before symptoms surfaces. If you can't get disposable, get those woven sewn with changeable non woven layers. It's available out there. Search for it.
2. WASH your hands and don't touch your MEN (mouth, eyes, nose)
3. Stay away from crowds pleaseeeee.  Limit your activities to need base. One month without social activities really won't kill anyone!
4. Oh right, I no longer use cash in China, it's all e-payment. So if you've handled cash, you know the drill ya. Sanitise! (your hands, not the cash)

It takes everyone to stop this from spreading. If you still think this is just a normal FLU and not taking any precautions, then you really are a danger to the community at this moment in time.

Peace out.  Stay calm.

=======================================================================

I am writing to you from Bergamo, Italy, at the heart of the coronavirus crisis. The news media in the US has not captured the severity of what is happening here. I am writing this post because each of you, today, not the government, not the school district, not the mayor, each individual citizen has the chance, today to take actions that will deter the Italian situation from becoming your own country’s reality.  The only way to stop this virus is to limit contagion. And the only way to limit contagion is for millions of people to change their behavior today.

If you are in Europe or the US you are weeks away from where we are today in Italy.

I can hear you now. “It’s just a flu. It only affects old people with preconditions”

There are 2 reasons why Coronavirus has brought Italy to it’s knees. First it is a flu is devastating when people get really sick they need weeks of ICU – and, second, because of how fast and effectively it spreads. There is 2 week incubation period and many who have it never show symptoms.

When Prime Minister Conte announced last night that the entire country, 60 million people, would go on lock down, the line that struck me most was “there is no more time.” Because to be clear, this national lock down, is a hail mary. What he means is that if the numbers of contagion do not start to go down, the system, Italy, will collapse.

Why? Today the ICUs in Lombardy are at capacity – more than capacity. They have begun to put ICU units in the hallways. If the numbers do not go down, the growth rate of contagion tells us that there will be thousands of people who in a matter of a week? two weeks? who will need care. What will happen when there are 100, or a 1000 people who need the hospital and only a few ICU places left?

On Monday a doctor wrote in the paper that they have begun to have to decide who lives and who dies when the patients show up in the emergency room, like what is done in war. This will only get worse.

There are a finite number of drs, nurses, medical staff and they are getting the virus. They have also been working non-stop, non-stop for days and days. What happens when the drs, nurses and medical staff are simply not able to care for the patients, when they are not there?

And finally for those who say that this is just something that happens to old people, starting yesterday the hospitals are reporting that younger and younger patients – 40, 45, 18, are coming in for treatment.

You have a chance to make a difference and stop the spread in your country. Push for the entire office to work at home today, cancel birthday parties, and other gatherings, stay home as much as you can. If you have a fever, any fever, stay home. Push for school closures, now. Anything you can do to stop the spread, because it is spreading in your communities – there is a two week incubation period – and if you do these things now you can buy your medical system time.

And for those who say it is not possible to close the schools, and do all these other things, locking down Italy was beyond anyone’s imagination a week ago.

Soon you will not have a choice, so do what you can now.

Please share.
